Meet & Greet
The free “meet and greet” will give you an opportunity to meet Dr. Santiago and ask questions about the practice and medical conditions we treat. You can choose for this visit to be virtual or in-person at the clinic.
Check your email after scheduling for an invitation to the patient portal. New patient forms, including the membership agreement, are available on the patient portal to review prior to your visit if desired.
